Yes, you need several items. The Iphone comes with a 2 year contract, mandatory internet service extra cost $29.95 a month, plus $69.99 a month minimum for the phone service, you also need to have good credit to be approved to get the service but not necessarily a credit card, plus whatever fees you will be charged to include starting service, taxes and fees.

ok I just got mine yesterday . As long as her debit card has a Visa or Master card logo on it you are fine .It’s 199 for an 8g model and 299 for a 16 g model. As far as the deposit goes it depends on her credit .If you have any questions email me.

if you are a new customer you do need a credit card. They need to see if you have bad credit (meaning they don’t think you’d be able to pay monthly) here is a story about deposit:
“Consumers who are found to have bad credit will be required to pay an additional amount in a security deposit in order to qualify for the service plan. AT&T does not publish its rate card, but the most-common amount reported by bad credit customers is $250 per line. Several consumers have reported to CreditCards.com and on the AT&T online forum they were asked for or charged $750, and some reports say the maximum is $1,000 per line”

the apple store doesn’t sell you the cell phone plan, only the device. the reason at&t requires a credit check is bc they are giving you the service before you pay for it every month. you are essentially financing your minutes. if you are only buying the device itself, a credit check is not required. go back to the store and tell the at&t rep that you have cash to pay the full price of the phone. they won’t turn you away.

Yes the Apple store does the same. You activate on iTunes.

But you can activate prepaid now on GoPhone plans.

if you are going to go to the store anyways after doing this form, you may as well just go to the store and have them do the credit check, because it only takes about 5 mins, and you will know right then and there weather or not you have to pay a deposit.

but to answer you, if it does not ask for your social security number, full name and address, then no. it is just gathering your information so you have it all in one place when you go to the actual store.

Yes when yu go to iTunes store go to the settings of your it we account. If you don’t know how toto iTunes store in itunes and then go to the top right corner where it says your account name and then click it and it will ask for your account password. Put in your password and there it will have a list of the ways you want to pay and you can change the way you wan to pay and you put in your credit card.
